As a member of the Kois Center, Dr. Bae uses Kois medical and dental history forms. These forms cover all aspects of dentistry and allow us to get an insight into your needs. You can download your forms today.

Please either print off, or fill out the following forms: Medical History, Dental History, Personal Information Forms, and send us via email or bring along with you to your dental appointment. This will provide us with important personal information about you, as well, we can contact your insurance provider to keep track of your dental coverage limits, and provide more accurate estimates.
